During this year and subsequent years the practice are required to take part in an important national project about diabetes care and treatment in the NHS. Patient information will be gathered once a year from our clinical system for those patients who have a confirmed diagnosis of diabetes.

The data will be used in order to plan diabetes care in response to local needs and to improve care for people with diabetes.
